Opinion: While companies promise a revolution in gut health monitoring, the current crop of AI stool trackers lacks the clinical validation to justify their steep price tags.

In the ever-expanding landscape of wellness trackers, the industry has finally arrived at the most private biosignal of all: the toilet bowl. As Wired first reported, the global gut health market is projected to reach nearly $106 billion by 2029, and a new wave of AI-powered hardware is attempting to turn bathroom breaks into data-driven reports. But the current value proposition of these devices is deeply suspect.

Two primary players are leading this charge. Kohler Health offers a device for $449, paired with a $130 annual family membership. Meanwhile, the startup Throne—which raised $10 million in July—sells a similar device for $399 with a $70 annual membership. Both products utilize cameras and light sensors to analyze urine and stool consistency and color, feeding the results into an algorithm that provides hydration and gut health scores via an app.

On the surface, the pitch is seductive: automated monitoring that removes the need for tedious manual journaling. Scott Hickle, Throne’s cofounder and CEO, argues that patients with digestive conditions or inflammatory bowel disease would benefit from this, noting that physician-requested stool logs suffer from nearly zero compliance because "no one wants to journal about their poop."

However, when we strip away the sleek hardware, what is the actual clinical utility? For the average healthy person, the answer seems to be "not much." Daniel Freedberg, a New York gastroenterologist and spokesperson for the American Gastroenterological Association, describes the technology as "overkill," noting that users can simply look at their own stool without the help of an expensive AI.

Companies argue that the value lies in long-term pattern recognition. Nora Tophof, managing director at Kohler Health, suggests that tracking can help users identify how specific medications or foods affect them. While pattern tracking is useful, it is hardly a medical breakthrough.

The real tension arises when these companies pivot toward diagnostic-adjacent claims. Kohler Health asserts its tracker can already alert users to the presence of blood. Throne is taking it further; Hickle claims a next-generation prototype using multispectral imaging will be more sensitive than the human eye at detecting blood, with a potential launch in two years. Hickle suggests this could help detect colorectal cancer—a critical goal given that the disease is now the leading cause of cancer death for those under 50—though he explicitly states, "We don't want to be in the business of making diagnoses."

This is where the "vaporware" alarm bells ring. If the goal is to detect blood to prompt a doctor's visit, the cost remains a massive barrier. Gianluca Ianiro, an assistant professor of gastroenterology at the Catholic University of Rome, notes that for a screening test to be effective, it needs to be cheap and non-invasive. At hundreds of dollars plus recurring fees, these devices fail the affordability test.

Beyond the cost, there is the predatory nature of the data model. Lauren Hendry Parsons of the Mozilla Foundation warns that users are essentially paying subscriptions to access their own biological information, creating a lock-in effect that makes it difficult for consumers to detach from the service.

Until these companies can provide rigorous, peer-reviewed evidence that their AI-driven "report cards" lead to better clinical outcomes than a simple visual check or a lab-grade test, these devices remain expensive novelties. We are being asked to pay a premium for the privilege of having a camera watch us go to the bathroom, all for insights that—for most of us—are already visible to the naked eye.
