Companies like Kohler Health and Throne are betting on AI-powered stool analysis, but gastroenterologists question if these devices offer any science beyond what a user can see with their own eyes.

The wellness tracking industry has expanded from heart rates and sleep cycles to the most private of biosignals: human waste. As the global gut health market projections climb toward nearly $106 billion by 2029, a new category of 'smart toilets' is attempting to monetize the bathroom break, as Wired first reported.

Two primary players are leading this charge. Kohler Health, an offshoot of the plumbing giant, offers a device for $449, paired with a $130 annual family membership. Meanwhile, startup Throne—which raised $10 million in July—sells its hardware for $399 with a $70 annual membership. Both products consist of white devices that clip to the toilet, utilizing downward-facing cameras and light sensors to record video of deposits. These images are then processed by algorithms to analyze color, consistency, and other metrics, delivering hydration and gut health scores via an AI-powered app.

From a clinical perspective, however, the value proposition is shaky. Daniel Freedberg, a New York gastroenterologist and spokesperson for the American Gastroenterological Association, suggests that for healthy individuals, these devices are essentially 'overkill.' According to reporting from Wired, Freedberg notes that users can simply look at their own stool and questions whether AI is contributing any actual new science to the process.

The companies argue that the value lies in longitudinal data rather than a single snapshot. Nora Tophof, managing director at Kohler Health, contends that long-term tracking allows users to identify personal patterns, such as weekend dehydration or reactions to specific medications. Scott Hickle, CEO and cofounder of Throne, argues the technology solves a compliance issue for patients with inflammatory bowel disease who are often asked by physicians to keep stool logs but find manual journaling tedious.

There is also a push toward diagnostic utility. Colorectal cancer is now the leading cause of cancer death for people under 50, and Throne intends to address this. Hickle states that a next-generation device, expected within two years, will use multispectral imaging to detect blood in fecal matter with greater sensitivity than the human eye. While Hickle clarifies that Throne does not intend to make formal diagnoses, he suggests the data should prompt users to escalate conversations with doctors. Kohler Health claims its current tracker already alerts users to the presence of blood.

Critics remain skeptical of the cost-to-benefit ratio. Gianluca Ianiro, an assistant professor of gastroenterology at the Catholic University of Rome, argues that screening tools must be both non-invasive and inexpensive, noting that these devices are not cheap.

Beyond the clinical utility, there are significant privacy concerns. Lauren Hendry Parsons, a digital rights advocate at the Mozilla Foundation, warns that the subscription model creates a scenario where users must pay repeatedly to access and analyze their own personal health information, making it difficult for consumers to detach from the ecosystem.
